Chloroquine is a malarial drug first discovered in 1934. It’s still in use for malaria management, although the most common species of malaria-causing organisms are long resistant to it. Hydroxychloroquine (Plaquenil) also has anti-malarial activity, but is much more commonly used to treat certain auto-immune disorders, including rheumatoid arthritis, and systemic lupus erythematosus. Both of these drugs are structurally related to quinine, the famous ‘Jesuit Bark’ (Chinchona spp.) that was the first effective treatment for malaria (and the chemical that gives tonic water its unique flavor). Both pharmaceuticals are long out of patent protection, so generic versions are widely available. Depending on locale, a typical month-long treatment with hydroxychloroquine in the developing world was about US $4.65. Both drugs are on the World Health Organization’s List of Essential Medicines.

All quinine-analog drugs have the potential for side effects. Some are mild (diarrhea, nausea, tinnitus) others can be quite serious (inflammation of the retina, anemia, cardiac instability). Chloroquine and hydroxychloroquine preferentially collect in the lungs, which helps increase potency with smaller doses. However, part of risk is due to the medications also collecting more in cells with melanin, which include skin and eye cells. Damage to the eyes can be a risk with large doses or extended use as a malaria or autoimmune disease treatment. People with cardiac problems may also be more at risk of adverse cardiac reactions. There is also a significant risk of interactions with other medications when taken simultaneously. Because of this, both chloroquine and hydroxychloroquine are available only by prescription.

Chloroquine and hydroxychloroquine have an extensive research basis as antivirals. There are two acknowledged mechanisms by which these drugs exert antiviral effects.

OBJECTIVE:

Assess the efficacy of a 10-week consumption of guava juice on the iron status of children with mild iron deficiency anemia.

METHODS:

Ninety-five boarding school children aged 6-9 years identified as anemic were randomly allocated to receive 300 mL of natural guava juice containing ∼200 mg of ascorbic acid (AA) or placebo (guava-flavored juice free of AA) with the main meal (5 d/wk). Information about dietary intake was collected at weeks 3, 5, and 7 at school and household levels. Changes in hemoglobin (Hb) and plasma ferritin (PF) among the subsample iron deficient at baseline (n = 33) were the main outcomes.

RESULTS:

Iron and phytic acid intakes at school and at home did not differ between groups. Baseline Hb and PF were 11.9 ± 0.5 g/dL and 8.2 ± 3.6 ng/mL for the guava, and 11.4 ± 1.1 g/dL and 7.4 ± 4.6 ng/mL for the placebo group (Hb: p = 0.08; PF: p = 0.31); at week 10 of the study, corresponding values were 13.1 ± 0.9 g/dL and 17.9 ± 10.3 ng/mL (n = 16), and 12.3 ± 1.3 g/dL and 15.4 ± 5.8 ng/mL (n = 12) (Hb: p = 0.05; PF: p = 0.21). With analysis of variance (ANOVA) for repeated measures, the guava group had 0.64 g/dL higher Hb (CI(95), 0.18-1.11; p = 0.01) and 2.47 ng/mL higher PF (CI(95), -1.04 to 5.98; p = 0.12) compared with the placebo group.

CONCLUSION:

Guava juice providing 200 mg AA at one meal on each school day had a marginal effect on Hb and PF concentrations in children consuming high-phytate diets fortified with iron.

BACKGROUND:

Iron deficiency anemia has occurred more frequently in female than male regular blood donors. Iron supplement is necessary for maintaining the hemoglobin level. A combination with ascorbic acid increases the absorption of iron.

OBJECTIVE:

Compare the effect of ferrous fumarate 200 mg/day and the combination of ferrous fumarate 200 mg/day with ascorbic acid 100 mg/day and 500 mg/day on hemoglobin level and red blood cell indices in female regular blood donors.

MATERIAL AND METHOD:

Female regular blood donor volunteers were randomly divided into three equal groups to supplement with each regimen for one month. Hemoglobin (Hb), MCV, MCH, and MCHC before and after blood donation at 0.5, 1, 2, and 3 months, were compared.

RESULTS:

Ninety-six volunteers were included and equally divided into three groups of 32 volunteers each. There were no significant differences between age, BMI, ABO blood group, Rh, Hb, MC, MCH, and MCHC before blood donation between each group (p > 0.05). The duration of recovery times of Hb to before blood donation levels in group I, II, and III being 2, 3, and 1 month, respectively were statistically significant difference (p < 0.001). The duration of recovery times of MCV, MCH, and MCHC to before blood donation levels in both group II and III were similar (0.5 months in every value), which was more rapid than in group I (> 3, 3, and 1 month, respectively) with statistically significant difference (p < 0.001). All three groups tolerated well. No participant withdrew from the present study because of side effects.

CONCLUSION:

The present study shows that a combination of ferrous fumarate 200 mg and ascorbic acid 500 mg per day accelerates timing of hemoglobin and red blood cell indices in recovery to the level of before blood donation in female regular blood donors.

CÉLKITŰZÉS

Annak megállapítása, hogy a 10 héten keresztül fogyasztott guavalé milyen hatékonyságot fejt ki azoknak a gyermekek a vas státuszára, akik enyhe vashiányos vérszegénységben szenvednek.

MÓDSZEREK:

Kilencvenöt bentlakásos iskolai gyermekről (6-9 évesek) állapították meg, hogy anémiásak, őket esetlegesen választották ki, hogy 300 ml természetes guavalét kapjanak, ami 200 mg aszkorbinsavat (AA) tartalmazott, vagy pedig placebót (guava ízesítésű dzsúszt, ami nem tartalmazott AA-t) főétkezéskor (heti 5 nap). Gyors felépülési idő a hemoglobinszint tekintetében rendszeres női (vér) donorok esetében, akik vasfumarátot és nagy mennyiségű aszkorbinsavat (C-vitamint) kaptak táplálék-kiegészítőként.

HÁTTÉR:

Vashiányos vérszegénység gyakrabban fordul elő rendszeres női véradók esetében, mint férfi donoroknál. A vaspótlás szükséges, hogy fenn lehessen tartani a vér hemoglobinszintjét. Aszkorbinsavval kombinálva a vas felszívódásának mértéke megnövekszik.

CÉLKITŰZÉS:

Hasonlítsuk össze a naponta fogyasztott 200 mg/nap vasfumarát hatását  a vasfumarát 200 mg/nap plusz 100 mg/nap aszkorbinsavval, valamint a napi plusz 500 mg aszkorbinsav fogyasztással kombinálva a vér hemoglobinszintjére és a vörös vérsejt mutatóira.   aszkorbinsavval kombinálják, az javítja a vas felszívódásának mértékét.

ANYAG ÉS MÓDSZER:

Önkéntes női véradókat esetlegesen osztottak szét három egyforma csoportra, hogy mindhárom táplálék-kiegészítést teszteljék rajtuk egy hónapos időtartam alatt. Összehasonlították a hemoglobinszintjüket (Hb), MCV, MCH, és MCHC-t a véradás előtt és után 0.5, 1, 2, and 3 hónappal.

EREDMÉNYEK:

Kilencvenhat önkéntes vett részt a kísérletben, őket három egyforma csoportra osztották fel, azaz 32 tagú önkéntes csoportokat vizsgáltak. Nem voltak jelentős különbségek életkor BMI, ABO vércsoport, Rh, Hb, MC, MCH, és MCHC tekintetében véradás előtt minden csoport között (p > 0.05). A hemoglobinszint visszanyerési ideje a véradás előtti szintre az I., II.  és III. csoportban 2, 3 és 1 hónap volt, jelentős statisztikai különbségek mutatkoztak (p < 0.001). Az MCV, MCH, és MCHC felépülési ideje a véradás előtti szintre mindkét csoportban (I., II.) azonos volt (fél hónap minden érték tekintetében), ami gyorsabb volt, mint az I. csoportban (> 3, 3, és 1 hónap), statisztikailag jelentős különbségeket mutatva (p < 0.001). Mindhárom csoport jól tűrte a kezelést. Egyetlen résztvevő se lépett vissza ettől a kísérlettől mellékhatásokra való hivatkozással.

KÖVETKEZTETÉS:

A jelenlegi kísérlet azt mutatja, hogy a 200 mg vasfumarát kombinációja 500 mg/nap aszkorbinsavval felgyorsítja  a hemoglobin és a vörös vérsejtek mutatóinak visszanyerését a véradás előtti szintre rendszeres női véradók esetében.
